Outpatient PT Services

If you or someone you love struggles with performing daily activities or participating in exercise or activities that are enjoyable because of imbalance, weakness, fatigue or pain, consider our physical therapy services.

Services are Offered In-Person in Your Home or Virtually/Telehealth

If you or someone you know has difficulty with activities such as: getting out of a chair, stepping up or down curbs or stairs, gardening, walking, or performing exercises or activities that are enjoyable without feelings of imbalance, weakness, fatigue or pain, consider seeking outpatient physical therapy services. A Physical Therapist can help to identify and address the underlying source of these issues and you improve your quality of life and ability to participate in activities that you enjoy!

We specialize in the evaluation, treatment and management of conditions such as:

  • Balance and falls
  • Vertigo and dizziness
  • Neurologic conditions such as dementia, stroke, Parkinson’s disease
  • Neuropathy and sensation deficits

  • General deconditioning and weakness after an illness or hospitalization
  • Oncology/cancer-related fatigue and deconditioning
  • COPD and other pulmonary conditions
  • Heart failure and other cardiac conditions

  • Total joint replacement prehab and rehab
  • Spinal stenosis, back or neck pain

Physical Therapy Service Details

Comprehensive Fall Risk Assessments: If you answer “Yes” to ANY of the following questions, contact us today about a detailed fall risk assessment.

1. Have you fallen in the last year?

2. Do you ever feel unsteady on your feet or have difficulty walking?

3. Are you afraid of falling?

4. Do you have difficulty rising from a chair without using your hands?

Balance and Vestibular Rehabilitation: 80% of adults > 65 years of age will experience some type of dizziness or imbalance at some point.  Some types of vertigo and dizziness can be improved with comprehensive assesssment and treatment by a Physical Therapist.  

Neurologic Rehabilitation: Neurologic conditions such as Parkinson’s disease, stroke, neuropathy and Multiple Sclerosis can cause functional strength, balance, gait and mobility deficits that make it difficult to enjoy regular activity and movement. We can help you improve in these areas and maximize your movement and mobility.

Pulmonary Rehabilitation: We offer individualized rehabilitation for adults with lung conditions such as COPD, lung cancer and pulmonary fibrosis. By focusing on improvement of shortness of breath, coughing, fatigue and activity participation, we can help you take control of your symptoms and your daily activities!

 

Cancer Rehabilitation:  Activity, movement and other physical therapy interventions can be important during cancer treatment and once in remission to help combat the effects of the treatment and illness.

Osteoporosis Management: Bone density may improve with the right type and amount of activity.  We focus on interventions to improve bone density, reduce risk for fracture and treatment after fractures such as hip fractures and spinal compression fractures.

Orthopedic Physical Therapy:  We can help you get moving again by reducing your symptoms of discomfort, swelling, stiffness and/or weakness that may be associated with an acute injury or a chronic condition.

Total Knee and Hip Replacement Prehab and Rehab: Being well prepared for a total joint replacement is key to success with post-surgery rehabilitation. We can help you improve your physical condition including strength, range of motion and balance and prepare your home set-up prior to the surgery. We can also help you during your post-surgical rehabilitation to help you get back to doing the things you enjoy doing as quickly as possible.

Rehabilitation and recovery after injury, surgery, hospitalization or illness: Muscle strength and physical function can decline during a hospitalization or time spent in bed while recovering from injury, surgery, or illness. Rehabilitation is an important part of your recovery from any acute health issues.
