What will I need prior to my first PT session?
- A PT referral from your medical provider
- A copy of your current insurance cards and ID
- A list of your current medications and health history

- We accept Medicare and Medicare Advantage plans
- We do not accept Medicaid, Fidelis, Worker’s Compensation or No Fault plans
- We are out of network with all insurances other than Medicare but will accept other insurances as an out of network provider. We will verify your out of network Physical Therapy coverage and discuss your benefits with you before scheduling your first session.
